Regard demonstrated leading performance in longitudinal chart comprehension, clinical accuracy, diagnosis identification and overall strategic value.

Ambience Healthcare performed strongly in documentation completeness, specialty fidelity and clinician usability. SmarterDx distinguished itself in acute-care evidence traceability, denial prevention, implementation and analytics. Waystar/Iodine led the acute clinical documentation improvement and coding-integrity measure, while Solventum showed particular strength in EHR interoperability, cybersecurity and enterprise data stewardship.

In the ambulatory and value-based-care assessment, Navina led measures involving longitudinal record comprehension, patient-specific evidence, HCC compliance and explainability.

Black Book asked users to assess the vendors using 18 weighted Key Performance Indicators across four domains:

The KPIs measure clinical accuracy and patient safety, longitudinal chart comprehension, patient-specific evidence traceability, documentation completeness, specialty fidelity, diagnosis specificity, coding integrity, denial prevention, HCC compliance, EHR interoperability, clinician usability, implementation, analytics, explainability, cybersecurity, vendor responsiveness and strategic value.

The assessment was developed from documented product capabilities, identifiable provider deployments, named EHR integrations, regulatory materials, case studies and independently available evidence reviewed through July 30, 2026. Vendor size, market visibility, sponsorship, promotional activity, report licensing and analyst-relations participation do not determine inclusion, weighting or ranking.

Evidence Traceability Becomes a Defining Requirement

Black Book found that the ability to reconstruct the evidence supporting a clinical AI recommendation is becoming a foundational enterprise requirement.

Healthcare organizations must be able to identify the laboratory findings, imaging, medications, prior documentation, structured data and current observations supporting a material diagnosis or documentation recommendation. This traceability is essential to clinician review, patient safety, coding compliance, payer defense, internal audit and ongoing model oversight.

"Clinical AI will increasingly be judged by whether it can explain why a recommendation exists, what evidence supports it and how it affects measurable clinical and operational outcomes," Brown said. "Clinical evidence must establish the diagnosis, the diagnosis must support the documentation, and the documentation must support coding and reimbursement. Financial objectives cannot determine what the clinical record says."

2027 Outlook

Black Book expects converged clinical AI to become an enterprise architecture and governance decision during 2027 rather than a series of isolated departmental purchases.

Health systems will increasingly evaluate how platforms operate across data ingestion, diagnosis support, documentation, coding, risk adjustment, clinical workflow and denial prevention.

The report anticipates increased competition from EHR-native AI, wider use of longitudinal record reasoning, stronger requirements for evidence traceability, more specialty-level validation, tighter controls over model changes and continued consolidation among clinical AI, EHR and revenue-cycle technology companies.
